Completing PPTC 326 – Statutory Declaration in Lieu of Guarantor

Form PPTC 326 (Statutory Declaration in Lieu of Guarantor) is used when you do not know anyone who meets the criteria of a guarantor to sign your application for travel document for non-Canadian.

Applying for or renewing a travel document can be straightforward until you find yourself without an eligible guarantor. That is where Form PPTC 326 (Statutory Declaration in Lieu of Guarantor) comes in. This form allows applicants to confirm their identity through a formal declaration when a standard guarantor is not available.

How to Complete Form PPTC 326 (Statutory Declaration in Lieu of Guarantor)

Section A. Applicant’s Personal Information

You must provide:

  • Full name (last name and given names)
  • UCI
  • Date of birth
  • Place of birth (include the city and country of birth)

Section B. Declaration of Applicant

This section contains your explanation for why you are unable to have an eligible guarantor sign your application.

You must:

  • Explain why you cannot find an eligible guarantor
  • List the details of two pieces of identification (type and number) that you presented to the official to verify your identity
  • Confirm that the information in your Travel Document application is true and accurate, including the date on the application and whether it is for yourself or your child

Section C. Declaration of Official

An authorized official fills out this section. Do not complete or sign the declaration before meeting with the official.
They must provide:

  • Full name
  • Occupation
  • Business address
  • Phone number

They must also confirm that you made the declaration in their presence (under oath or solemn declaration), then sign and date the form, including the city and province.

Who Can Sign Form PPTC 326?

Only the following may complete and sign the statutory declaration:

  • Lawyer
  • Notary public
  • Justice of the peace
  • Commissioner for Oaths

Additional Requirements for the Statutory Declaration

The official must:

1. Sign the back of one passport photo and write:

“This is Exhibit A to the Statutory Declaration of (Name) declared before me on (Date) at (City, Province).”

2. Certify your ID copies

They must confirm that the copies are true copies of the originals they saw.
They must also write the date and location where the originals were viewed and sign each copy.
