- No income or zero income: You or a family member had no income in a certain period, and OSAP requires verification.

How to Write an OSAP Affidavit

An OSAP affidavit can take the form of an affidavit or statutory declaration, generally divided into: title, introductions, body paragraphs, conclusion, your signature, the appropriate jurat and the notary's seal and signature.

- In The Matter of the OSAP Application of [Your Full Name]

Similar to all affidavits or statutory declarations, it should read: I, [Your Full Name], of the City of [City], in the Province of Ontario, make oath and say as follows (or "solemnly declare as follows" if making a statutory declaration):

This is where you explain your situation with relevant details. Be clear, specific, and truthful. It is numbered consecutively.
Example:
- I have been separated from my spouse, [name of spouse], since [date of separation]
2. Since our separation, I have been solely responsible for the care of my children, namely:
- [name of child], born on [child's birth date]
- [name of child], born on [child's birth date]
3. I will continue to care for and support my children throughout my studies. They shall reside with me at [Your Address]

And I make this Affidavit conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath and by virtue of the Canada Evidence Act.

The affidavit must be signed in the presence of a notary public or commissioner for oaths. DO NOT sign it in advance. The commissioner will also sign and stamp it with their seal.
