What is an Affidavit in Lieu of a Birth Certificate?

An affidavit or statutory declaration in lieu of a birth certificate, also known as a statutory declaration of age, is a sworn statement about your birth, which can be used in the place of your birth certificate.

When is it used?

It is used when your birth certificate is lost, and a replacement cannot be ordered in time for the intended purpose.

What does it contain?

Your affidavit in lieu of a birth certificate must contain the following information:

  1. Your date of birth
  2. Your place of birth, including the hospital
  3. Your parents, if known
  4. Explanation as to why your original birth certificate is not available
  5. Explanation as to why a replacement certificate is not available and cannot be obtained in time.

It is also advisable to support your affidavit with the sworn statement of another person who has firsthand information about your birth.
